Access to health care including access to specialists

Objective 1: Sustain recruitment

Continue recruitment and development of efforts for healthcare providers and specialists.

Objective 2: Facility expansion

Expand the square footage of Jersey Community Hospital to accommodate growing demand and allow for extra space for additional specialties.

Objective 3: Improve patient navigation through healthcare system

Implement a patient navigation system to guide individuals through the healthcare system, ensuring they receive appropriate care and follow-up.

 

Chronic disease including obesity

Objective 1: Chronic Care Management

Strengthen the existing Chronic Care Management program by continuing to incorporate personalized care plans, regular check-ins, and patient education to manage chronic conditions effectively.

Objective 2: Community education to provide health lifestyle resources and information.

Jersey Community Hospital will continue nutrition counseling and wellness center offerings as well as regular workshops on healthy eating, physical activity, and stress management to empower community members in making positive lifestyle changes.

Objective 3: Collaborative care approach

Foster collaboration between primary care providers, specialists, nutritionists, and fitness professionals to create comprehensive care plans for patients dealing with chronic diseases and obesity.

 

Mental health and addiction

Objective 1: Integrate mental health services within primary care settings to ensure a holistic approach to patients’ well-being and early identification of mental health concerns.

Jersey Community Hospital continues to screen for mental health concerns and has now established annual screening for substance use issues to help identify risky use and get patients the help they need as soon as possible.

Objective 2: Continue public campaigns to reduce stigma surrounding mental health and addiction, encouraging individuals to seek help and support for themselves or their loved ones.

Through the use of a Health Resources and Services Administration, or HRSA, grant, that Jersey Community Hospital received in August of 2021 there will be a new public campaign to reduce the stigma of addiction and encourage community support for those that need assistance.

Objective 3: Bring awareness to onsite LCSW counseling sessions, medication-assisted recovery treatment clinic, and jail-based treatment programs.

Jersey Community Hospital is partnering with the Jersey County Sheriff’s Department, Probation Department, and Pre-Tail Services to provide Medication Assisted Recovery Services in the jail to help those that are incarcerated start their recovery journey. This is facilitated by a new Learning Collaborative that these agencies have entered into with the Illinois Department of Human Services.
